Two years into the engagement, the client has held the number one spot on Google for a large share of its tracked keywords. Not as a temporary spike, but as a sustained position competitors have not been able to dislodge.
The Client
A single-location medical spa competing in a crowded local market: Botox and dermal fillers, body contouring, laser hair removal, HydraFacial and dermaplaning, and a full menu of skin treatments.
Med spas live and die by "near me" searches, and this market had no shortage of competitors chasing the same handful of terms.
The Challenge
With dozens of overlapping services, the client needed to rank for a wide spread of keywords at once, not just one or two flagship terms.
Losing visibility on even a handful of high-intent searches like "coolsculpting near me" or "botox [city]" means losing the patient before they ever pick up the phone.
The Approach
The work was built around how patients actually search: by treatment name, by "near me," and by city.
-
Build a dedicated page for every core service
Each treatment got its own optimized page instead of being buried in a combined services list, so Google had a clear page to rank for each one.
-
Target the exact language patients use
On-page and local SEO work aimed at treatment names, "near me" queries, and city-specific terms, the three patterns that make up almost all med spa search.
-
Track a portfolio, not a headline keyword
Rather than optimizing for a single keyword per page, we tracked and refined against a full portfolio of 118 keywords spanning every service line the client offers.
-
Keep refining month over month
Ongoing on-page and local SEO work, so positions are defended and rebuilt as competitors move, which is what turned a good quarter into two sustained years.
The Results
Two years in, the numbers hold across the whole keyword portfolio, not just the flagship terms.
| Position band | Keywords | Share of portfolio |
|---|---|---|
| In the top 10 | 114 | 96.6% |
| In the top 5 | 108 | 92% |
| Holding the #1 spot | 29 | 25% |
| Still unranked | 3 | 2.5% |
Bars share one scale, with headroom left for the label. Bands overlap: every #1 keyword is also counted in the top 5 and top 10.
96.6% of tracked keywords rank in the top 10 (114 of 118), and more than half rank in the top 3. The average rank across all 118 keywords is 3.0, meaning the typical tracked search places the client at or near the very top of the page.
108 of 118 keywords (92%) hold a top-five position, including 29 keywords in the number one spot. Those #1 rankings span every major service line, including Botox, dermal fillers, Sculptra, CoolSculpting, CoolTone, laser hair removal, HydraFacial, and microneedling, not just a single lucky keyword.
Only 3 of 118 keywords remain unranked, down from a much larger starting gap when tracking began.
Every Keyword Holding the #1 Spot
The number one rankings aren't clustered around one lucky phrase. They span the client's entire menu of services, which is exactly why the traffic keeps compounding instead of depending on a single keyword holding its position.

What This Looks Like in Real Traffic
Rankings only matter if they turn into visits, and Google's own Search Console data confirms they are. Over the trailing three months, the site has averaged:
| Metric | Monthly average | Three-month total |
|---|---|---|
| Organic clicks | ~690 | 2,080 |
| Search impressions | ~68,700 | 206,000 |
Roughly 690 organic clicks a month, and about 68,700 monthly search impressions, meaning the site's listings are showing up in front of tens of thousands of local searchers every month.
Translating Rankings Into Patients
Position on the page determines how much of that traffic actually converts into a click, and how much of that click turns into a patient. As a rule of thumb:
| Position | Share of clicks on that search |
|---|---|
| #1 | ~20% |
| #2 | ~15% |
| #3 | ~10% |
| #4 | ~6–7% |
| #5 | ~2% |
Rankings past the fifth spot are left out of this estimate entirely. They exist, but including them would inflate the numbers without adding anything reliable.
With 108 of the 118 tracked keywords sitting inside that top-five window, and a conservative 1% site conversion rate applied on top (some months trend closer to 2% to 3%), the math lines up with what shows up on the client's own booking calendar: an estimated 70 to 80 new patient inquiries a month traceable to organic search.
This is a conservative, directional estimate built from published click-through benchmarks and the client's real ranking and traffic data, not a precise attribution model. Even treated conservatively, it points to the same conclusion: consistent top-five visibility across a wide keyword portfolio is producing a steady, real stream of new patients every month, not just better numbers on a rank tracking report.
Why It Matters
Patients rarely search for a med spa by name. They search by treatment.
"Botox near me." "Coolsculpting cost near me." "Laser hair removal [city]." Ranking first for that many of those searches, and holding that position for two years, means the client shows up first at the exact moment someone is ready to book.
That kind of consistency compounds. It's not one good month of rankings, it's two years of patients finding this med spa before they find anyone else.
